What Happens Next

Once contact is made, the referral and intake process follows these steps.

Step 1: Initial Contact

Reach out by phone, email, or secure contact form. Our intake team responds within 24–48 hours.

Step 2: Eligibility Screening

We help determine service availability, county eligibility, insurance status, and current care connections. Missing paperwork is okay.

Step 3: Matching of Services

If eligible, we connect individuals to housing, care coordination, ECM, or stabilization services. If we’re not the right fit, we provide warm referrals.

Our intake process is personal, not clinical. It usually begins with a brief call or meeting where we take time to understand your current situation, your history, and what you hope will change. Together, we identify the services that make the most sense for you and outline clear next steps. Even when full enrollment isn’t immediate, we create a short-term plan so you leave the conversation with clarity and direction.

If You’re a Family Member or Loved One

You don’t need permission to contact us. Many families reach out first to ask questions or get guidance during a difficult time. We can help you understand options, prepare your loved one for next steps, and offer support as you navigate the process.
