WHAT IS ECM

A Human-First Approach to Care Managemnt

Enhanced Care Management (ECM) is a Partnership HealthPlan program that provides whole-life development services in Northern California for individuals with complex medical, behavioral, substance use disorder, and social needs.

Our ECM program is person-centered and system-smart. Each client is paired with a dedicated Care Manager who serves as an advocate, problem-solver, and consistent guide. Care is grounded in relationships and strengthened by deep knowledge of healthcare, housing, behavioral health, and justice systems.

You are not expected to navigate this alone. We help make sense of the system and move things forward with you.

Who Qualifies for ECM

ECM is available to Partnership HealthPlan recipients facing serious health and social challenges, including:

Chronic physical or mental health conditions
Recent homelessness or ongoing housing instability
Frequent emergency room visits or hospital stays
Substance use or behavioral health challenges
Involvement with the justice system
Difficulty with activities of daily living (ADLs)

These individuals need more than appointments or referrals. They need a coordinated plan and consistent support. ECM makes that possible.

How to Get Started with ECM

There are several ways to begin ECM services at New Life Discovery:

  • Individuals with Partnership HealthPlan may contact us directly or request a referral through their managed care plan
  • individuals, family members or services providers may call our ECM intake team to discuss eligibility
  • Current NLDP clients may transition into ECM through their housing or care team

We handle coordination with your health plan so you don’t have to manage paperwork, phone calls, or delays. When help is needed, it should be compassionate, immediate, and effective.
